Physiotherapist

Responsibilities

• Triage referrals via telephone
• Assess patient needs and coordinate appropriate care pathways
• Facilitate timely discharges
• Support patients transitioning from hospital to home
• Utilize clinical judgement for safe and effective care planning

Requirements

• HCPC registration
• Bachelor’s degree in Physiotherapy or a related field
• Strong clinical reasoning and decision-making skills
• Excellent communication and interpersonal abilities
• Ability to work independently and as part of a team

Preferred Qualifications

• Experience in community or discharge planning settings
