Physiotherapist

Responsibilities

• Facilitate discharge planning and coordinate patient movement from acute wards to onward services
• Assess patients and identify specific discharge needs
• Work closely with multidisciplinary teams including nursing, occupational therapists, and medical staff
• Manage complex cases to ensure safe discharge with appropriate support
• Maintain accurate patient records and liaise with external healthcare providers
• Contribute to a 7-day service, including weekends and public holidays

Requirements

• HCPC registration as a Physiotherapist
• Degree in Physiotherapy or equivalent
• Extensive discharge experience within an acute hospital setting
• Experience as a Senior Physiotherapist
• Ability to work in a multi-disciplinary team
• Enhanced DBS check required

Preferred Qualifications

• Previous experience working within the NHS
