Physiotherapist

Responsibilities

• Deliver expert physiotherapy care to patients in the community, specialising in aged care, frailty, and falls management
• Conduct detailed assessments and develop personalised rehabilitation plans
• Provide ongoing support to improve patients' mobility and independence
• Prescribe and manage appropriate equipment to support patient care
• Collaborate with other healthcare professionals and agencies for coordinated patient management
• Communicate with patients and families to educate them on conditions and treatment options
• Maintain accurate patient records and contribute to service development

Requirements

• HCPC registration as a Physiotherapist
• Right to work in the UK
• Relevant degree in Physiotherapy or equivalent qualification
• Experience in community aged care, frailty, falls management, equipment prescription, and rehabilitation
• Strong clinical assessment and treatment planning skills
• Ability to work independently and within a multidisciplinary team
• Excellent communication and interpersonal skills
