Responsibilities

• Deliver comprehensive physiotherapy services within virtual wards and community settings
• Conduct thorough patient assessments
• Develop and implement individualized treatment plans
• Monitor patient progress
• Collaborate with a multidisciplinary team to ensure holistic care
• Provide education and support to patients and their families
• Contribute to the continuous improvement of physiotherapy services

Requirements

• Degree in Physiotherapy
• HCPC registration
• Experience in community physiotherapy, preferably within virtual wards
• Strong clinical skills and knowledge in relevant areas
• Excellent communication and interpersonal skills
• Ability to work effectively within a multidisciplinary team
• Commitment to ongoing professional development
