Physician Associate

Responsibilities

• Provide frontline care to patients experiencing homelessness, behavioral health challenges, and complex chronic conditions
• Manage diverse patient panels by delivering acute, chronic, and preventative care in the field
• Conduct patient assessments, physical exams, and order/interpret diagnostics
• Design and implement care plans to support patient stability
• Collaborate with a multidisciplinary team including physicians, nurses, and social workers
• Document all clinical encounters for continuity of care

Requirements

• 2+ years of experience working with CalAIM populations (homelessness, SMI/SUD, or transitioning from incarceration)
• Experience in street medicine or related community-based clinical settings
• Active, unrestricted California Physician Assistant License
• Successful completion of the Physician Assistant National Certificate Exam
• Valid CA Driver’s License and reliable, insured vehicle
• Ability to travel up to 75% of work time
• National Provider Identifier (NPI) and DEA number
• Basic Life Support (BLS) Certification

Preferred Qualifications

• Experience with diverse and underserved populations
• Familiarity with value-based care models
• Bilingual abilities (Spanish or other languages)
• Local Bay Area residency
