Founded in 2022, Nest utilizes a family-based, in-home, and virtual care model including primary adult and pediatric care, mental health, and nutrition support.

Responsibilities

  • Manage a panel of families as the primary care provider to improve whole-family health through longitudinal primary and preventive care
  • Practice with the support of an interdisciplinary team including MDs, LCSWs, RNs, and psychiatrists
  • Utilize electronic health records and care management tools to document interactions and track tasks
  • Attend daily team huddles and weekly case conferences to advise on physical health needs
  • Propose treatments for chronic and infectious diseases, prescribe medications, and educate patients
  • Order, administer, obtain, and record diagnostic tests
  • Provide advocacy, navigation support, and education to families regarding health and social care services
  • Participate in outreach efforts to explain program details to potential families

Requirements

  • Completion of Physician Assistant or Nurse Practitioner accredited program
  • Active, unrestricted license to practice in Louisiana
  • At least 2 years of experience providing clinical services to pediatric (including newborns) and adult patients
  • Experience with chronic medical and behavioral health conditions
  • Proficiency in clinical nursing skills such as blood drawing (venipuncture & phlebotomy)
  • Current CPR certification (BLS)
  • Valid driver's license, reliable transportation, and auto insurance
  • Ability to lift up to 25 pounds and travel to family homes
  • Availability for a full-time, five-day work week including a rotating call schedule

  • Experience leading an interdisciplinary care team
  • Experience working in a startup environment
