Founded in 2022, Nest utilizes a family-based, in-home, and virtual care model including primary adult and pediatric care, mental health, and nutrition support.
Responsibilities
- Manage a panel of families as the primary care provider to improve whole-family health through longitudinal primary and preventive care
- Practice with the support of an interdisciplinary team including MDs, LCSWs, RNs, and psychiatrists
- Utilize electronic health records and care management tools to document interactions and track tasks
- Attend daily team huddles and weekly case conferences to advise on physical health needs
- Propose treatments for chronic and infectious diseases, prescribe medications, and educate patients
- Order, administer, obtain, and record diagnostic tests
- Provide advocacy, navigation support, and education to families regarding health and social care services
- Participate in outreach efforts to explain program details to potential families
Requirements
- Completion of Physician Assistant or Nurse Practitioner accredited program
- Active, unrestricted license to practice in Louisiana
- At least 2 years of experience providing clinical services to pediatric (including newborns) and adult patients
- Experience with chronic medical and behavioral health conditions
- Proficiency in clinical nursing skills such as blood drawing (venipuncture & phlebotomy)
- Current CPR certification (BLS)
- Valid driver's license, reliable transportation, and auto insurance
- Ability to lift up to 25 pounds and travel to family homes
- Availability for a full-time, five-day work week including a rotating call schedule

- Experience leading an interdisciplinary care team
