Founded in 2022, Nest is on a mission to make comprehensive medical, behavioral, and social care radically accessible to America’s highest risk families through primary adult and pediatric care, mental health, and nutrition support.

Responsibilities

  • Manage a panel of families as the primary care provider for longitudinal primary and preventive care
  • Deliver care in home settings and via telehealth/virtual care modalities
  • Collaborate with an interdisciplinary team including MDs, LCSWs, RNs, and dieticians
  • Utilize electronic health records to document member interactions and track tasks
  • Propose treatments for chronic and infectious diseases and prescribe appropriate medications
  • Order, administer, and record diagnostic tests
  • Provide advocacy, navigation support, and education regarding health and social care services
  • Participate in outreach efforts to explain program details to potential families

Requirements

  • Completion of an accredited Physician Assistant or Nurse Practitioner program
  • Active, unrestricted license to practice in Arizona
  • At least 2 years of experience providing clinical services to pediatric and adult patients
  • Experience with chronic medical and behavioral health conditions
  • Proficiency in clinical nursing skills such as venipuncture and phlebotomy
  • Current CPR certification (BLS)
  • Valid driver’s license, reliable transportation, and auto insurance
  • Ability to lift up to 25 pounds

Preferred Qualifications

  • Experience leading an interdisciplinary care team
  • Experience working in a startup environment
