Nest is the first-of-its-kind family-based, in-home, and virtual care model. Founded in 2022, Nest is on a mission to make comprehensive medical, behavioral, and social care radically accessible to America’s highest risk families through primary adult and pediatric care, mental health, and nutrition support.
Responsibilities
- Manage a panel of families as the primary care provider for longitudinal primary and preventive care
- Deliver care in home settings and via telehealth/virtual care modalities
- Collaborate with an interdisciplinary team including MDs, LCSWs, RNs, and dieticians
- Utilize electronic health records to document member interactions and track tasks
- Propose treatments for chronic and infectious diseases and prescribe appropriate medications
- Order, administer, and record diagnostic tests
- Provide advocacy, navigation support, and education regarding health and social care services
- Participate in outreach efforts to explain program details to potential families
Requirements
- Completion of an accredited Physician Assistant or Nurse Practitioner program
- Active, unrestricted license to practice in Arizona
- At least 2 years of experience providing clinical services to pediatric and adult patients
- Experience with chronic medical and behavioral health conditions
- Proficiency in clinical nursing skills such as venipuncture and phlebotomy
- Current CPR certification (BLS)
- Valid driver’s license, reliable transportation, and auto insurance
- Ability to lift up to 25 pounds

- Experience leading an interdisciplinary care team
- Experience working in a startup environment
