Nest is the first value-based care provider built for families, on a mission to make comprehensive medical, behavioral, and social care radically accessible to America’s highest risk families. Founded in 2022, Nest utilizes a family-based, in-home, and virtual care model to provide primary adult and pediatric care, mental health support, and chronic condition management.

Responsibilities

• Provide clinical services to pediatric (including newborns) and adult patients in their homes
• Manage both chronic medical and behavioral health conditions
• Perform clinical nursing skills including blood drawing (venipuncture & phlebotomy) and patient assessments
• Reinforce care plans for conditions such as diabetes, respiratory concerns, hypertension, CHF, and depression
• Utilize independent clinical decision-making and problem-solving to manage patient care
• Participate in a rotating call schedule including occasional evenings and weekends
• Educate and encourage vaccination against COVID-19

Requirements

• Completion of an accredited Physician Assistant or Nurse Practitioner program
• Active, unrestricted license to practice in Arizona
• At least 2 years of experience providing clinical services to pediatric and adult patients
• Current CPR certification (BLS)
• Valid driver’s license, reliable car, and auto insurance
• Ability to lift up to 25 pounds and carry portable healthcare supplies
• Ability to work a full-time, five-day work week

Preferred Qualifications

• Experience working as the leader of an interdisciplinary care team
• Experience working in a startup environment
