Physician

Responsibilities

• Provide frontline care to patients experiencing homelessness, behavioral health challenges, and complex chronic conditions
• Manage diverse patient panels by delivering acute, chronic, preventative, and palliative care in the field
• Conduct patient assessments, physical exams, and prescribe treatments within California scope of practice
• Collaborate with a multidisciplinary team including nurses and social workers to coordinate care
• Design and implement care plans to support patient stability and health outcomes
• Document all encounters to maintain accurate medical records

Requirements

• MD or DO degree
• Active, unrestricted California Physician & Surgeon license
• 2+ years of clinical experience with underserved populations
• Experience in street medicine or community-based settings
• Valid California Driver’s License and access to a reliable, insured vehicle
• Ability to travel up to 75% of work time
• DEA registration and NPI number
• Basic Life Support (BLS) certification

Preferred Qualifications

• Board certification in Family Medicine, Internal Medicine, Emergency Medicine, Palliative Care, Geriatrics, or Addiction Medicine
• Familiarity with value-based care models
• Bilingual abilities (Spanish or other languages)
• Local knowledge of Bay Area neighborhoods and community partners
