Physical Therapy Assistant

Responsibilities

• Implement physical therapy treatment plans in collaboration with a supervising Physical Therapist
• Reinforce home exercise programs by educating patients, families, and caregivers
• Monitor and communicate changes in patient status, progress, or goals to the supervising PT
• Educate patients and caregivers on proper body mechanics and safe mobility techniques
• Document timely, accurate clinical notes in the electronic medical record
• Coordinate care by communicating effectively with the interdisciplinary team
• Support daily operations through flexible scheduling and travel within the agency territory

Requirements

• Associate’s degree in Physical Therapy
• Registration and good standing with the New Hampshire Board of Nursing
• At least 2 years of hospital or clinic experience (homecare experience preferred)
• Current CPR Certification
• Valid Driver's License
• Ability to meet criminal background and motor vehicle record requirements
