About the role
Responsibilities
- Deliver skilled PTA visits in the home under the supervising PT’s plan of care
- Execute treatment plans focusing on functional mobility, gait/balance, transfers, and strengthening
- Set and track objective progress and communicate updates to the supervising PT
- Drive multidisciplinary coordination and escalate changes in condition
- Coach patients and caregivers with practical home programs
- Document promptly and thoroughly in Homecare Homebase (HCHB)
Requirements
- Active Colorado PTA license in good standing
- Strong clinical judgment and ability to work independently in the field
- Current driver’s license and comfort with daily travel across assigned territory
- Ability to carry and transport basic therapy equipment
- Competency in core home health PTA scope including safety-first mobility and fall prevention
Preferred Qualifications
- Prior home health experience
- Experience with Homecare Homebase (HCHB) or similar home health EMR
- Clinical experience in high-signal ortho/neuro populations
