About the Company

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our mission is to dramatically improve patient access to home health care through clinical excellence and technology that reduces documentation burden.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care
• Execute treatment plans focusing on functional mobility, gait/balance, transfers, and strengthening
• Set and track objective progress and communicate updates to the supervising PT
• Drive multidisciplinary coordination and escalate changes in patient condition
• Coach patients and caregivers with practical home programs
• Document promptly and thoroughly in Homecare Homebase (HCHB)

Requirements

• Active Texas PTA license in good standing
• Strong clinical judgment and ability to work independently in the field
• Current driver’s license and comfort with daily travel across assigned territory
• Ability to carry and transport basic therapy equipment
• Competency in safety-first mobility work, strengthening, and home safety assessments

Preferred Qualifications

• Prior home health experience
• Experience with Homecare Homebase (HCHB) or similar home health EMR
• Clinical experience in high-signal ortho/neuro populations
• Strong organization and time management skills
