Physical Therapist

Responsibilities

• Provide physical therapy services for Home Health visits in the Canton and Needham areas
• Conduct patient assessments and implement treatment plans in a home setting
• Maintain accurate documentation and patient records
• Coordinate with preceptors and educators during the initial training period

Requirements

• Valid Massachusetts Physical Therapist license in good standing
• Proven experience in Home Health settings
• Proficiency with Oasis E documentation
• Willingness to travel within assigned territories (including areas such as Arlington, Newton, Wellesley, Dedham, and Braintree)
