Responsibilities

• Provide high-quality physical therapy services for home health visits within the Dorchester teams
• Conduct patient assessments and implement individualized treatment plans in a home setting
• Maintain accurate documentation and clinical records
• Collaborate with the interdisciplinary care team to ensure optimal patient outcomes

Requirements

• Valid Massachusetts Physical Therapist (PT) license in good standing
• Proven experience in Home Health settings
• Proficiency with Oasis E documentation
• Willingness to travel within the assigned territory
