Physical Therapist

Responsibilities

• Accept and manage patients within a designated geographic area
• Evaluate patients and develop individualized plans of care focused on restoring function and independence
• Implement customized treatment programs aligned with physician orders and patient goals
• Communicate effectively with patients, families, physicians, and internal care team members
• Complete timely, accurate documentation for all visits
• Participate in discharge planning and patient education
• Maintain compliance with all clinical and documentation standards

Requirements

• Physical Therapist Program Graduate
• TN Physical Therapist License or eligibility
• Liability insurance
• CPR/First Aid certification

Preferred Qualifications

• Home health experience
