About the role
Responsibilities
- Evaluate patient needs for physical therapy and develop appropriate treatment plans
- Provide care and treatment based on physician care plans, agency policy, and professional standards
- Instruct family members, PTAs, Home Health Aides, and other caregivers in physical therapy techniques
- Provide coverage across various clinic locations in Eastern Suffolk
Requirements
- Licensed Physical Therapist in the state of New York or eligible
- Mandatory certifications in First Aid, AED, and CPR
- Ability to travel to multiple clinic locations within the region
