About the role
Responsibilities
- Provide 1:1 physical therapy treatment to residents within the facility
- Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
- Deliver care in accordance with professional standards and the established plan of care
- Lead a weekly group exercise class focused on mobility and functional wellness
- Collaborate with facility staff and caregivers to support patient outcomes
- Plan and implement therapy programs to improve or restore functional abilities
- Perform and document initial evaluations, progress notes, and discharge summaries
- Monitor patient progress and modify treatment plans as needed
- Provide care with a strong understanding of geriatric and memory care populations
Requirements
- Physical Therapist Program Graduate
- Licensed Physical Therapist in the state of New York or eligible
