Responsibilities

• Provide physical therapy to adult and geriatric patients in their homes across the Chicago, IL area
• Participate in structured clinical pathways and ongoing mentorship beyond orientation
• Coordinate with a responsive support team to optimize daily scheduling and reduce administrative burden
• Utilize company-provided tablet and EMR systems to document patient care

Requirements

• Active Physical Therapist license in good standing (new graduates encouraged)
• BLS CPR certification
• Reliable transportation to travel between patient homes
• Basic comfort with technology including email, texting, and EMR
