Responsibilities

• Provide physical therapy to adult and geriatric patients in their homes across the Chicago area
• Follow clinical pathways and participate in ongoing mentorship and coaching
• Manage a flexible schedule with autonomy over daily patient visits
• Utilize provided tablet, data plan, and supplies for documentation and care delivery

Requirements

• Active Physical Therapist license in good standing (new graduates encouraged)
• BLS CPR certification
• Reliable transportation for travel between patient homes
• Basic comfort with technology, including email, texting, and EMR systems
