Physical Therapist - Home Health

Responsibilities

• Develop and implement physical therapy plans of care based on patient assessments and physician direction
• Integrate physical therapy into the total Home Health plan of care and coordinate services with team members
• Evaluate Hospice patients for palliation of symptoms that restrict mobility, transfers, and activities of daily living

Requirements

• Graduation from a certified Physical Therapy program
• Currently licensed in the State of Washington
• Current CPR card
• Reliable transportation and automobile liability insurance
• Valid WA State Driver's License

Preferred Qualifications

• LSVT BIG certification
• Previous Home Care or in/outpatient experience
