About the role

Responsibilities

  • Deliver high-quality, patient-centered rehabilitation services directly in the home setting
  • Develop individualized therapy interventions to restore function, reduce pain, and promote optimal health
  • Evaluate patient progress through observation and measurable outcomes, adjusting plans as needed
  • Collaborate with physicians and the homecare team to refine treatment plans
  • Design and teach home exercise programs to patients, families, and caregivers
  • Document measurable goals and clinical services in the electronic health record
  • Supervise Physical Therapist Assistants (PTAs) in accordance with scope of practice
  • Perform timely therapy reassessments using objective, measurable data

Requirements

  • Licensed Physical Therapist in New Hampshire
  • Bachelor's degree in Physical Therapy
  • Two or more years of hospital or clinic experience
  • CPR certification
  • Satisfactory criminal and motor vehicle record
  • Ability to travel within the agency's territory in all seasons

Preferred Qualifications

  • Master's or Doctorate in Physical Therapy
  • Previous homecare experience
