Physical Therapist

Responsibilities

• Provide physical therapy services to patients in their homes based on a physician's plan of care
• Assess and evaluate therapeutic, rehabilitative, and functional status
• Evaluate the home environment and assess muscle strength, mobility, gait, and ROM
• Direct physical therapy treatments and determine priority needs
• Instruct patients and caregivers in the use and care of therapeutic appliances
• Report patient reactions to treatment or changes in condition to physicians

Requirements

• Current unrestricted PT license
• CPR certification
• Knowledge of computerized documentation and office software
• Clean background and criminal record

Preferred Qualifications

• Minimum one year of home care experience
• Three to five years of clinical experience
• Previous supervisory experience
• Experience with OASIS
