About the role
Responsibilities
- Implement individualized treatment plans including therapeutic exercise, gait training, balance activities, and functional mobility tasks
- Observe, document, and report participant progress, functional changes, and safety concerns to the supervising PT
- Coordinate with nurses, social workers, occupational therapists, and other PACE team members
- Provide therapy services in participant homes, the PACE day center, and clinic environments
- Train participants and caregivers on safe use of assistive devices and mobility equipment
- Complete accurate, timely electronic health record (EHR) documentation
Requirements
- Accredited PTA Program graduate
- Active California PTA license
- Current CPR/BLS certification
- Ability to travel locally and work independently
Preferred Qualifications
- Experience working with geriatric populations
- Experience in long-term care, home health, or PACE settings
