Physical Therapist Assistant

Responsibilities

• Implement individualized treatment plans including therapeutic exercise, gait training, balance activities, and functional mobility tasks
• Observe, document, and report participant progress, functional changes, and safety concerns to the supervising PT
• Coordinate with nurses, social workers, occupational therapists, and other PACE team members
• Provide therapy services in participant homes, the PACE day center, and clinic environments
• Train participants and caregivers on safe use of assistive devices and mobility equipment
• Complete accurate, timely electronic health record (EHR) documentation

Requirements

• Accredited PTA Program graduate
• Active California PTA license
• Current CPR/BLS certification
• Ability to travel locally and work independently

Preferred Qualifications

• Experience working with geriatric populations
• Experience in long-term care, home health, or PACE settings
