Physical Therapist

Responsibilities

• Perform physical therapy evaluations and develop individualized treatment plans
• Provide therapeutic exercises, training, and mobility support to clients in their homes
• Educate clients and their families on safety protocols and home exercise programs
• Maintain accurate and timely clinical documentation
• Collaborate with interdisciplinary team members to ensure high-quality care

Requirements

• Active New Hampshire Physical Therapist license
• Strong assessment and clinical reasoning skills
• Strong interpersonal and communication skills
• Passion for helping clients achieve functional goals and independence

Preferred Qualifications

• Previous experience in home health settings
