About the role
Responsibilities
- Own the caseload as a case manager, accountable for therapy plans, progression, and outcomes
- Complete comprehensive in-home assessments and deliver skilled PT visits
- Build and execute plans of care including gait/balance, transfers, and fall prevention
- Set measurable goals, track progress, and adjust treatment plans
- Drive multidisciplinary coordination and communicate changes in condition to the care team
- Manage treatment and outcomes, including PTA supervision where appropriate
- Coach patients and caregivers with practical home programs
- Document promptly and thoroughly in Homecare Homebase (HCHB)
Requirements
- Active Texas PT license in good standing
- Strong clinical judgment and ability to work independently in the field
- Current driver's license and comfort with daily travel across assigned territory
- Ability to carry and transport basic therapy equipment
- Competency in core home health PT scope (transfers, gait training, balance, home safety)
Preferred Qualifications
- Prior home health experience
- Experience with Homecare Homebase (HCHB) or similar home health EMR
- Experience supervising or mentoring PTAs
- Strong organization and time management skills
