Physical Therapist

Responsibilities

• Own the caseload as a case manager, accountable for therapy plans, progression, and outcomes
• Complete comprehensive in-home assessments and deliver skilled PT visits
• Build and execute plans of care including gait/balance, transfers, and fall prevention
• Set measurable goals, track progress, and adjust treatment plans
• Drive multidisciplinary coordination and communicate changes in condition to the care team
• Manage treatment and outcomes, including PTA supervision where appropriate
• Coach patients and caregivers with practical home programs
• Document promptly and thoroughly in Homecare Homebase (HCHB)

Requirements

• Active Texas PT license in good standing
• Strong clinical judgment and ability to work independently in the field
• Current driver's license and comfort with daily travel across assigned territory
• Ability to carry and transport basic therapy equipment
• Competency in core home health PT scope (transfers, gait training, balance, home safety)

Preferred Qualifications

• Prior home health experience
• Experience with Homecare Homebase (HCHB) or similar home health EMR
• Experience supervising or mentoring PTAs
• Strong organization and time management skills
