Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our mission is to dramatically improve patient access to home health care through mission-driven work and cutting-edge technology designed to reduce documentation burden.

Responsibilities

• Own the caseload as a case manager, accountable for the patient’s therapy plan, progression, and outcomes
• Complete comprehensive in-home assessments and deliver skilled PT visits
• Build and execute a clear plan of care including gait/balance, transfers, strengthening, and fall prevention
• Set measurable goals, track objective progress, and adjust treatment plans
• Drive multidisciplinary coordination by communicating changes in condition to the care team
• Manage treatment and outcomes directly, including PTA supervision where appropriate
• Coach patients and caregivers with practical home programs
• Document promptly and thoroughly in Homecare Homebase (HCHB)

Requirements

• Active Texas PT license in good standing
• Strong clinical judgment and comfort working independently in the field
• Current driver’s license and comfort with daily travel across assigned territory
• Ability to carry and transport basic therapy equipment
• Competency in core home health PT scope including safety-first mobility and home safety assessments

Preferred Qualifications

• Prior home health experience
• Homecare Homebase (HCHB) or similar home health EMR experience
• Experience supervising or mentoring PTAs
• Strong organization and time management skills
