Physical Therapist

Responsibilities

• Perform comprehensive assessments in the patient's home
• Complete skilled documentation through Homecare Homebase
• Evaluate patients and perform appropriate therapy interventions
• Develop a comprehensive plan of care and coordinate multi-disciplinary care
• Perform prompt clinical documentation and visit notes
• Maintain communication with physicians, care team members, patients, and families
• Implement and maintain physical therapy protocols
• Manage patient treatment and outcomes directly or through PTA supervision

Requirements

• Current Texas Physical Therapist (PT) license
• One year of physical therapy experience
• Current Texas driver's license
• CPR certification
• BLS Certification

Preferred Qualifications

• Experience with Homecare Homebase
• Previous Home Health experience
