About the role
Responsibilities
- Perform comprehensive assessments in the patient's home
- Complete skilled documentation through Homecare Homebase
- Evaluate patients and perform appropriate therapy interventions
- Develop a comprehensive plan of care and coordinate multi-disciplinary care
- Perform prompt clinical documentation and visit notes
- Maintain communication with physicians, care team members, patients, and families
- Implement and maintain physical therapy protocols
- Manage patient treatment and outcomes directly or through PTA supervision
Requirements
- Current Texas Physical Therapist (PT) license
- One year of physical therapy experience
- Current Texas driver's license
- CPR certification
- BLS Certification
Preferred Qualifications
- Experience with Homecare Homebase
- Previous Home Health experience
