Physical Therapist

Responsibilities

• Own the caseload as a case manager, managing therapy plans, progression, and outcomes
• Complete comprehensive in-home assessments and deliver skilled PT visits
• Build and execute plans of care focusing on gait, balance, transfers, and fall prevention
• Drive multidisciplinary coordination by communicating changes in condition to the care team
• Coach patients and caregivers with practical home programs
• Document promptly and thoroughly in Homecare Homebase (HCHB)

Requirements

• Active Texas PT license in good standing
• Strong clinical judgment and ability to work independently in the field
• Current driver’s license with comfort with daily travel across assigned territory
• Ability to carry and transport basic therapy equipment
• Competency in core home health PT scope including mobility, strengthening, and home safety

Preferred Qualifications

• Prior home health experience
• Experience with Homecare Homebase (HCHB) or similar home health EMR
• Experience supervising or mentoring PTAs
• Clinical experience in high-signal PT populations
