Responsibilities

• Carry a full, high-quality caseload and manage patients as a case manager
• Complete comprehensive in-home assessments and deliver skilled PT visits
• Build and execute clear plans of care including gait/balance, transfers, and strengthening
• Set measurable goals, track progress, and adjust treatment plans
• Drive multidisciplinary coordination and communicate changes in condition
• Manage treatment and outcomes, including PTA supervision where appropriate
• Coach patients and caregivers with practical home programs
• Document promptly and thoroughly in Homecare Homebase (HCHB)

Requirements

• Active Michigan PT license in good standing
• Strong clinical judgment and comfort working independently in the field
• Current driver’s license and comfort with daily travel across assigned territory
• Ability to carry/transport basic therapy equipment
• Competency in core home health PT scope (safety-first mobility, strengthening, and home safety)
• Strong communication and documentation skills

Preferred Qualifications

• Prior home health experience
• Experience with Homecare Homebase (HCHB) or similar home health EMR
• Experience supervising/mentoring PTAs
• Deeper clinical experience in high-signal PT populations
