Responsibilities
- Carry a full, high-quality caseload and manage patients as a case manager
- Complete comprehensive in-home assessments and deliver skilled PT visits
- Build and execute clear plans of care including gait/balance, transfers, and strengthening
- Set measurable goals, track progress, and adjust treatment plans
- Drive multidisciplinary coordination and communicate changes in condition
- Manage treatment and outcomes, including PTA supervision where appropriate
- Coach patients and caregivers with practical home programs
- Document promptly and thoroughly in Homecare Homebase (HCHB)
Requirements
- Active Michigan PT license in good standing
- Strong clinical judgment and comfort working independently in the field
- Current driver’s license and comfort with daily travel across assigned territory
- Ability to carry/transport basic therapy equipment
- Competency in core home health PT scope (safety-first mobility, strengthening, and home safety)
- Strong communication and documentation skills
Preferred Qualifications
- Prior home health experience
- Experience with Homecare Homebase (HCHB) or similar home health EMR
- Experience supervising/mentoring PTAs
- Deeper clinical experience in high-signal PT populations
