Pharmacist

Responsibilities

• Review, verify, and dispense prescribed medication orders for appropriateness
• Identify, resolve, and prevent drug-related problems
• Monitor drug therapy for effectiveness, toxicity, and adherence to evidence-based guidelines
• Conduct medication reconciliation and follow up on medication incidents
• Provide direct or functional supervision to technician staff
• Respond to drug information questions and promote safe medication practices

Requirements

• BSc in Pharmacy or PharmD degree
• Certificate of competency from the Pharmacy Examination Board of Canada
• Active registration with the Ontario College of Pharmacists (Part A)
• Certificates for smoking cessation and flu vaccination injection programs
• Knowledge of specialty medication dispensing and out-of-province dispensing processes
• Experience with HealthWatch Delta paperless workflow and POS systems
• Strong interpersonal, communication, and customer service skills

Preferred Qualifications

• Hospital Outpatient Pharmacy practice experience
• Hospital pharmacy residency
• Experience handling blister packs and pharmacy inventory management
