About the role
Responsibilities
- Review and dispense medication orders for appropriateness
- Identify, resolve, and prevent drug-related problems
- Monitor drug therapy for effectiveness, toxicity, and adherence to evidence-based guidelines
- Conduct medication reconciliation
- Follow up on medication incidents and promote safe medication practices
- Respond to drug information questions and provide supervision to technician staff
Requirements
- Bachelor's of Science in Pharmacy or Doctor of Pharmacy degree
- Certificate of competency from the Pharmacy Examination Board of Canada
- Active registration with the Ontario College of Pharmacists (Part A)
- Experience in an acute-care hospital or ambulatory pharmacy setting
- Strong interpersonal, communication, and organizational skills
Preferred Qualifications
- Hospital pharmacy residency certificate
- Experience with patient-centered care in surgery or oncology
- Membership in the Canadian Society of Hospital Pharmacists (CSHP)
- Faculty of Pharmacy University appointment
