About the role

Responsibilities

  • Review and dispense medication orders for appropriateness
  • Identify, resolve, and prevent drug-related problems
  • Monitor drug therapy for effectiveness, toxicity, and adherence to evidence-based guidelines
  • Conduct medication reconciliation
  • Follow up on medication incidents and promote safe medication practices
  • Respond to drug information questions and provide supervision to technician staff

Requirements

  • Bachelor's of Science in Pharmacy or Doctor of Pharmacy degree
  • Certificate of competency from the Pharmacy Examination Board of Canada
  • Active registration with the Ontario College of Pharmacists (Part A)
  • Experience in an acute-care hospital or ambulatory pharmacy setting
  • Strong interpersonal, communication, and organizational skills

Preferred Qualifications

  • Hospital pharmacy residency certificate
  • Experience with patient-centered care in surgery or oncology
  • Membership in the Canadian Society of Hospital Pharmacists (CSHP)
  • Faculty of Pharmacy University appointment
