Pharmacist

Responsibilities

• Review, verify, check and dispense all prescribed medication orders for appropriateness
• Identify, resolve and prevent drug related problems
• Monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelines
• Conduct medication reconciliation
• Follow up on medication incidents and promote safe medication practices on the unit
• Respond to drug information questions
• Provide direct and/or functional supervision of technician staff
• Perform other duties consistent with the job classification

Requirements

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree
• Certificate of competency from the Pharmacy Examination Board of Canada
• Active registration with the Ontario College of Pharmacists (Part A)
• Certificate of competency from an accredited smoking cessation and flu vaccination injection program
• Familiarity with specialty medication dispensing and out of province processes
• Experience working within a multi-disciplinary team
• Knowledge of adherence programs including compliance packaging and refill management
• Experience with HealthWatch Delta and Points of Sales Systems
• Experience handling pharmacy inventory

Preferred Qualifications

• Hospital Outpatient Pharmacy practice experience
• Hospital pharmacy residency
• Experience handling blister packs
