About the role
Responsibilities
- Function as a core clinical team member of the Calvary interdisciplinary team
- Review medication histories and assess appropriateness of current medications
- Monitor therapy relative to pain and other symptom management
- Provide education to patients and families regarding pharmacologic and non-pharmacologic interventions
- Recommend and implement risk mitigation strategies to optimize safe medication use
- Assist in the development of protocols and clinical pathways to improve palliative care
- Engage in didactic and experiential interprofessional palliative education
- Represent the team locally, regionally, and nationally through presentations
Requirements
- Active New York State pharmacy license in good standing
Preferred Qualifications
- Two years of post-graduate residency/fellowship training or equivalent experience in oncology, hospice, or geriatrics
- Board Certification by Board of Pharmacy Specialties or American Society of Pain Educators certification
