Pharmacist-Outpatient, Registry

Responsibilities

• Provide clinical and distributive pharmacy services for patients at the Outpatient Retail Pharmacy in Hyde Park and the Pharmacy Call Center in Burr Ridge
• Ensure the accuracy and appropriateness of medication orders by evaluating drug selection, dose, frequency, safety, and potential drug interactions
• Supervise pharmacy technicians working under your direction
• Monitor drug utilization and compliance with clinical pathways and IV to PO conversion
• Clarify medication orders with prescribing physicians and suggest alternative therapies when appropriate
• Document clinical interventions to prevent drug-related problems or promote cost-effective therapy
• Participate in pharmacokinetic dosing services and serum drug level monitoring

Requirements

• Graduate of an ACPE accredited School of Pharmacy
• Registered Pharmacist Licensure in the state of Illinois (or eligibility for same)
• Two years of hospital pharmacy experience in an organized healthcare setting
• Board Certification in an area of expertise (e.g., BCPS, BCOP, BCACP) required within 18 months of hire
• Strong written and verbal communication skills
• Ability to interact constructively with supervisors, peers, and support personnel

Preferred Qualifications

• Doctor of Pharmacy (PharmD) or another advanced degree
• Completion of an ASHP accredited residency
