Pharmacist

Responsibilities

• Provide progressive and comprehensive pharmaceutical services to hospital patients and the retail pharmacy
• Work closely with healthcare providers to ensure safe medication practices and positive patient outcomes
• Fill prescriptions for KVH Hospice under a collaborative practice agreement using approved algorithms

Requirements

• Bachelor’s Degree in Pharmacy
• Current license to practice Pharmacy in the State of Washington
• Familiarity with Automated Dispensing Cabinetry (e.g., Pyxis)
• Current CPR card
• Current PALS card

Preferred Qualifications

• Two years of work experience in a hospital setting
• Experience with Amerisource Bergen or Cerner
• Experience with CPOE, Emar, or Rx Informatics
• Experience with Bar Code at the Bedside
• Retail Pharmacy experience
• Knowledge of the 340B Program
• Prior experience with Hospice protocols and guidelines
