Pharmacist I - Outpatient

Responsibilities

• Review, interpret, and validate outpatient prescriptions to ensure accuracy and identify therapeutic incompatibilities
• Accurately prepare, label, dispense, and deliver medications in compliance with DOH regulations and internal standards
• Provide patient counseling on medication usage, dosage, potential side effects, and adherence
• Maintain and update patient medication records and profiles accurately and in a timely manner
• Monitor and manage medication inventory, including controlled substances and expiry tracking
• Ensure compliance with infection control, patient safety protocols, and quality standards
• Collaborate with physicians and healthcare teams to support safe prescribing practices

Requirements

• Bachelor's degree in Pharmacy
• Valid Department of Health (DOH) license
• Clinical exposure, internship, or placement in a hospital and/or retail pharmacy setting
• Strong knowledge of pharmaceutical procedures and current best practices
• Good understanding of pharmacy-related regulations
• Proficiency in Microsoft Office suite
