Pharmacist I

Responsibilities

• Review, verify, and dispense outpatient prescriptions in accordance with DOH, JCI, and hospital standards
• Provide accurate patient counseling regarding medication use, dosing, precautions, and storage
• Ensure timely dispensing to maintain optimal patient flow and meeting waiting-time targets
• Identify and resolve therapeutic duplications, drug interactions, and other medication-related issues
• Coordinate with physicians and nursing staff to clarify and optimize medication orders
• Support medication safety initiatives, including LASA management and high-alert medication guidelines
• Maintain accurate inventory control, monitor expiry dates, and ensure proper storage conditions
• Document clinical interventions and contribute to quality and performance improvement activities

Requirements

• Bachelor's degree in Pharmacy
• Valid DOH license
• Knowledge of pharmaceutical procedures and current best practices
• Strong understanding of pharmacy-related regulations
• Proficiency in Microsoft Office suite

Preferred Qualifications

• 1–2 years of experience in an outpatient or hospital pharmacy setting
