Pharmacist

Responsibilities

• Dispense prescription medications accurately in compliance with prescriber orders and pharmacy laws
• Compound medications according to professional standards and organizational policy
• Provide patient-centered medication counseling on dosage, administration, and side effects
• Monitor medication therapy for safety, effectiveness, and adherence
• Maintain compliance with controlled substance regulations (DEA, PA Board of Pharmacy)
• Collaborate with physicians, nurses, and behavioral health providers to support coordinated care
• Maintain accurate pharmacy documentation and records within the EHR

Requirements

• Active Pennsylvania Pharmacist License
• Active Controlled Substance Registration (DEA)
• Strong knowledge of prescription dispensing and verification
• Proficiency in dosage calculations and pharmaceutical compounding
• Knowledge of Pennsylvania and federal pharmacy laws
• Strong clinical judgment and attention to detail

Preferred Qualifications

• Retail, outpatient, or clinical pharmacy experience
• Experience in community health centers or FQHCs
• Familiarity with EHR and pharmacy management systems
