Pharmacist Clinical

Responsibilities

• Provide clinical pharmacy services within the scope permitted by law and organizational authorization
• Manage formulary medication processes and support non-formulary medication requests
• Coordinate with network pharmacies for dispensing approved medications
• Ensure adequate medication supply for patient transfer, release, or removal
• Collaborate with medical staff to identify medication-related needs for acute and chronic care
• Maintain accurate medication documentation in the electronic health record (EHR)
• Participate in patient care, quality improvement, and risk management activities
• Comply with emergency response policies and procedures
• Protect patient privacy and confidentiality

Requirements

• Current, full, active, and unrestricted pharmacy license in Texas or ability to obtain one
• Recognized as a Licensed Supervised Provider able to work under a CP/PAA
• Basic Life Support (BLS) certification
• B.S. in Pharmacy or Pharm D degree from an accredited program
• Ability to pass a medical examination and background investigation
• U.S. citizenship and five-year U.S. residency

Preferred Qualifications

• Experience in a detention, correctional, or residential healthcare setting
• Proficiency in Spanish
