Personal Care Partner/QMAP

Responsibilities

• Provide assistance with activities of daily living (ADLs) including bathing, dressing, grooming, and mealtime support
• Supervise and administer prescribed medications following the "five rights"
• Monitor resident status and report changes in hygiene, skin concerns, or physical needs to nursing leadership
• Maintain accurate documentation of resident care and medication administration (MARs)
• Respond to family questions with professionalism and warmth
• Maintain a safe environment and report suspected abuse or neglect immediately

Requirements

• Current state-approved medication aide/assistant certification (QMAP-A/L, Med Tech, Med Aide, or Nurse Delegation)
• High school diploma or equivalent (GED)
• Ability to read, write, and speak English
• Flexibility to work PRN shifts including days, evenings, nights, weekends, and holidays
• Ability to handle objects up to 50 pounds occasionally

Preferred Qualifications

• One year of experience in elder care, senior living, or home care
• Proficient computer skills
