Personal Care Partner II QMAP

Responsibilities

• Assist residents with personal care needs and activities of daily living (ADLs), including bathing, dressing, grooming, and meal assistance
• Administer medications in accordance with state law and facility protocols, ensuring the "5 Rights" are met
• Monitor and document resident health conditions, reporting any changes or concerns to the Director promptly
• Anticipate and identify resident care needs through careful observation and communication
• Maintain accurate medication records and restock medication carts and storage areas as needed
• Maintain resident confidentiality and handle sensitive information with professionalism

Requirements

• Medication certification (QMAP-A/L, Med Tech, Med Aide, or Nurse Delegation)
• Ability to read, write, and speak the English language
• Ability to lift up to 50 lbs occasionally and handle moderate physical activity
• Strong self-management, organization, and multi-tasking skills
• Proficiency with computers preferred
• One year of experience in elder care preferred
