Personal Care Partner II

Responsibilities

• Provide personal care services to residents, including assistance with bathing, dressing, meals, and laundry.
• Assist residents with prescribed daily medication routines in accordance with state law.
• Monitor and document resident conditions, health status, and safety.
• Identify and report changes in resident status or potential care needs to supervisors.
• Maintain open communication with peers and supervisors to ensure high-quality, person-centered care.
• Respond to the needs of family members and maintain resident confidentiality.
• Fulfill a minimum of 2 shifts per month on a flexible, as-needed (PRN) basis.

Requirements

• Medication certification (QMAP-A/L, Med Tech, Med Aide, or Nurse Delegation).
• High school diploma or GED.
• Fluency in English (reading, writing, and speaking).
• Ability to lift up to 50 pounds occasionally and perform moderate physical activity.
• Ability to work flexible shifts, including 12-hour shifts.

Preferred Qualifications

• One year of experience in elder care.
• Proficient computer skills.
• Strong self-management, organization, and multi-tasking abilities.
