About the role

Responsibilities

  • Attend to the daily living and safety needs of participants in the PACE center and occasionally in the home
  • Meet participant personal hygiene and basic needs in a timely and caring manner
  • Assist participants with activities of daily living, such as feeding and ambulation
  • Prepare and plate food for meal service following specific dietary instructions
  • Collect data and record participant care information accurately
  • Maintain an orderly and clean participant care unit
  • Observe participants for emotional, behavioral, and physical changes and report to nursing staff
  • Perform light housekeeping and homemaking services as outlined in care plans
  • Accompany participants to outside appointments

Requirements

  • Completion of accredited vocational nursing assistant course, hospital, or comparable participant care experience
  • High School diploma or GED
  • Knowledge of the PACE program
  • Minimum of one year experience working with the frail or elderly
  • Ability to work independently and exercise judgment
  • Physical strength to assist participants with mobility challenges
  • Valid CA driver’s license, personal transportation, and auto insurance
  • Current CPR/BLS certification (or ability to obtain within 30 days)
  • Proof of medical clearance for communicable diseases and up-to-date immunizations

Preferred Qualifications

  • Bilingual in Spanish
