About the role

Responsibilities

  • Conduct community outreach to individuals with SMI in homes, community locations, and care settings
  • Support health needs by scheduling appointments, taking vitals, and assisting with telehealth access
  • Build trust and relationships to empower individuals to pursue personal recovery goals
  • Serve as the primary point-of-contact for ongoing connection to supportive services
  • Address social determinants of health by facilitating access to benefits, resources, and transportation
  • Assist with care coordination and advocate for individual goals
  • Provide timely and thorough documentation of encounters
  • Perform initial and ongoing telephonic outreach

Requirements

  • Lived experience with SMI and/or SUD
  • State Peer Recovery certification or eligibility to obtain within 6 months
  • Two years of demonstrated recovery time from a mental health and/or substance abuse disorder
  • Valid driver’s license in good standing with ability to operate a vehicle
  • Proficiency with smartphone and computer technology
  • Ability to work Monday - Friday, 8:30 am - 4:30 pm

Preferred Qualifications

  • Experience with direct in-person outreach for individuals with SMI and SUD
  • Expertise in recovery education and coaching
  • Understanding of trauma-informed approaches to peer support
  • Familiarity with local community resources and behavioral health systems
  • Community Health Worker (CHW) Credential
