About the role
Responsibilities
- Conduct community outreach to individuals with SMI in homes, shelters, hospitals, and emergency departments
- Support health needs by scheduling appointments, taking vitals, and assisting with telehealth access
- Build trust and empower individuals to pursue personal recovery goals
- Serve as the primary point-of-contact for ongoing engagement in supportive services
- Facilitate access to benefits, resources, and transportation to address social determinants of health
- Assist with care coordination and advocate for individual goals
- Provide thorough documentation of encounters and participate in case reviews
- Perform initial and ongoing telephonic outreach
Requirements
- Lived experience with SMI (serious mental illness) and/or SUD (substance use disorder)
- State Peer Recovery certification or willingness to obtain within six months
- Two years of demonstrated recovery time from a mental health and/or substance abuse disorder
- Valid driver’s license in good standing and ability to operate a vehicle
- Proficiency with smartphone and computer technology
- Ability to work Monday - Friday, 8:30 am - 4:30 pm
Preferred Qualifications
- Experience with direct in-person outreach for individuals with SMI and SUD
- Expertise in recovery education and coaching
- Understanding of trauma-informed approaches to peer support
- Familiarity with local community resources and behavioral health systems
- Community Health Worker (CHW) Credential
