Patient Navigator

Responsibilities

• Oversee caseload of STD clinic patients seeking HIV prevention services
• Ensure patients are connected to prevention and treatment services and adhere to care plans
• Deliver health education and promotion messages according to clinical protocols
• Coordinate logistics including appointment reminders, scheduling, and medication adherence
• Collaborate with STD clinic staff to identify qualified program candidates
• Perform rapid HIV screening and provide confidential results and advising
• Maintain liaison with healthcare providers and public/private agencies to expedite patient follow-up

Requirements

• Baccalaureate degree with 12 semester credits in health education, health, social, or biological sciences; OR
• Baccalaureate degree with six months of full-time experience in health promotion or disease intervention
• High school diploma with four years of full-time experience in health promotion or disease intervention
• Equivalent combination of education and experience

Preferred Qualifications

• Experience as a Public Health Adviser, Assignment Level I (for Assignment Level II)
