Patient Navigator

Responsibilities

• Oversee a caseload of STD clinic patients seeking HIV prevention services
• Ensure patients are connected to prevention and treatment services and adhere to care plans
• Deliver health education and promotion messages according to clinical protocols
• Coordinate logistics including appointment reminders, scheduling, and medication adherence
• Assist patients with insurance enrollment, entitlements, and referrals to behavioral health teams
• Collaborate with clinic staff to identify qualified program candidates
• Maintain liaison with healthcare providers and public/private agencies to expedite patient follow-up

Requirements

• Baccalaureate degree with 12 semester credits in health, social, or biological sciences
• OR Baccalaureate degree with 6 months experience in health promotion or disease intervention
• OR High school diploma with 4 years of relevant experience
• At least one year of experience as a Public Health Adviser, Assignment Level I (for Level II)
