Patient Care Coordinator

Responsibilities

• Ensure efficient day-to-day operations of clinical areas within the disease site team
• Lead continuous improvement and project management practices to strengthen clinic processes
• Perform financial and administrative responsibilities to support effective site operation
• Contribute to staff development by promoting a collaborative learning environment for nurses, students, and ancillary staff
• Collaborate with the Nurse Manager to perform human resources responsibilities
• Liaise with the multi-disciplinary team to ensure high standards of quality and optimal patient care outcomes

Requirements

• BScN (Baccalaureate in Nursing) or Diploma in Nursing
• Current licensure/certificate of competency from the College of Nurses of Ontario (CNO)
• Minimum of three years of nursing experience
• Proven experience working in a supervisory capacity
• Basic Cardiac Life Support (BCLS) certification
• Strong leadership, decision-making, and problem-solving skills

Preferred Qualifications

• Master’s degree in Nursing
• Experience with project management or Lean process improvement
