Patient Care Coordinator

Responsibilities

• Act as the central connection point for patients, providers, and the clinic team
• Coordinate appointments to maximize provider utilization and minimize schedule gaps
• Maintain accurate patient records and manage follow-up tasks through the Athena tickler system
• Guide patients through care pathways and execute Transfer of Care processes
• Respond promptly to patient inquiries via phone, email, or other channels
• Support accurate billing processes, insurance verification, and patient intake coordination
• Protect patient confidentiality in accordance with HIPAA standards
• Maintain clinic operational standards and environment expectations

Requirements

• High school diploma or equivalent
• Minimum 2 years of experience in healthcare coordination, patient services, or medical office administration
• Experience with scheduling systems or electronic medical records (EMR)
• Strong organizational skills and attention to detail
• Excellent interpersonal and communication skills
• Ability to reliably commute to Brookline, MA

Preferred Qualifications

• Experience in behavioral health or mental health clinic environments
• Familiarity with Dr.Chrono, Athena EMR, or similar healthcare systems
• Experience coordinating care across interdisciplinary healthcare teams
• Knowledge of insurance verification and medical billing processes
