Patient Care Coordinator

Responsibilities

• Guide patients through care pathways and coordinate appointments to maximize provider utilization
• Maintain accurate patient records and manage follow-up tasks through the Athena tickler system
• Respond promptly to patient inquiries via phone, email, and other communication channels
• Collaborate with clinical providers, billing, and treatment coordinators to ensure coordinated care
• Protect patient rights and maintain HIPAA compliance regarding medical and financial information
• Support clinic operational standards, insurance verification, and patient intake coordination
• Identify workflow challenges and recommend process improvements to enhance clinic efficiency

Requirements

• High school diploma or equivalent
• Minimum 2 years of experience in healthcare coordination, patient services, or medical office administration
• Experience with scheduling systems or electronic medical records (EMR)
• Strong organizational skills and ability to manage multiple tasks in a fast-paced environment
• Excellent interpersonal and communication skills
• Ability to reliably commute to DTLA

Preferred Qualifications

• Experience in behavioral health or mental health clinic environments
• Familiarity with Dr.Chrono, Athena EMR, or similar healthcare systems
• Knowledge of insurance verification, patient intake workflows, or medical billing processes
• Experience coordinating care across interdisciplinary healthcare teams
